Victim Profile Questionnaire

This questionnaire is designed to help survivors safely share their experiences in a structured, secure way. Answers provide enough information for professional users to identify relevant connections while maintaining appropriate privacy protections. The questionnaire consists of eight sections.

Before starting, consider whether you want a particular photo to represent your profile. We recommend that this photo does not show your face for safety and identity protection reasons. However, if you feel safe in use of a personal image, you are welcome to upload one here. If you do not choose one, we will assign a random image to ensure your profile is easily identifiable to Professionals.
Section 2 provides space for your to summarize your experience and goals in your own words.

- Brief Overview* Provide a concise summary (2-3 paragraphs) of your experience. This helps professionals quickly understand if your case might be relevant to their work.

- Description of Your Offender If you would like to provide a physical description of your offender to aid Professional users in connecting cases, do so here. These field is optional.

  Note: For legal protection, we advise against naming individuals unless they’ve been convicted in court. Focus on descriptive patterns and institutional affiliations.
- Perpetrator Job, Role or Position* If you know your perpetrator’s occupation, role or position, provide that information. If you do not know or prefer not to say, simply write “Unknown.”
- Institutional Affiliation Although this field is not required, it is helpful if you can identify your offender’s affiliation with any large entities, organizations or institutions.
- Method or Approach If relevant, describe any notable patterns in how the crime or incident occurred. This information can help identify connections to other cases. Examples: He drove past me first, then came back and offered a ride. He used a flashlight in my eyes so I could not see his face.

- Current Living Situation* Which of the following describes your current living situation? Like Household Income, this data helps Not Just One and associated researchers understand service and justice gaps based on living situation.
- Language Select the primary or multiple language(s) you speak. This data helps Not Just One and researchers identify language barriers that may affect access to services and resources. Not Just One is also eager to ensure we serve our Survivors in the languages they speak, read and write.
- Family and Dependents* How many children are you parent or guardian for that live in your household (aged 17 or younger only)? Not Just One and researchers seek this data to provide insights into our Survivors’ priorities and challenges. We can also use it to tailor our offerings to better serve our Survivors.
- Political Affiliations How would you describe your political viewpoints? This data helps Not Just One and researchers identify service and justice gaps based on politics.
- Section 8: Editing Options and Confirmation
- Editing Options
  Once approved, your profile is fully editable. You may edit it at any time after it is activated. When you receive notification from Not Just One that your profile is active, please review it to ensure you did not include information that compromises your safety or creates legal vulnerability. Remember, because you are using a pseudonym, you are not directly identifiable through the profile itself. Only Professional users can view your profile. These users are bound by strict rules for viewing and engagement with our Survivors. They are not authorized to publish content about you without communicating with you first and receiving your approval. If you note any inappropriate use of the platform by our Professional users, it is critical that you contact Support (support@notjustone.org). Any Professional users breaking the rules of use and engagement are immediately removed from the platform.
